Please notify me of medical conditions/severe allergies diagnosed by your child’s physician.
Parents will provide all appropriate foods for their child with severe food allergies; i.e. snack time and class parties. You are welcome to send a sealed container with food items to keep in the classroom for the year for those unexpected party snacks. Children with known food allergies will not be allowed to eat any other snacks at school. They may, however, take the party snack home and let parents determine if it is a safe food item for them.
A child with a temp of 100 dg F or greater will be sent home from the clinic.
It is advisable that children remain fever free for at least 24 hours without the use of fever reducing medications before returning to school.
This will reduce the possible spread of viral and bacterial infections to students and staff.
A child vomiting at school will be sent home from the clinic.
Please keep children at home if they have had vomiting throughout the night or the morning before coming to school to prevent spread of possible viral illesses.
Bacterial conjunctivitis (pink eye) is very contagious. Matting, thick yellow-green drainage, puffiness and redness to one or both eyes are symptoms to watch for.
Please have a plan in place to have your child picked up from the clinic if you are unavailable. Due to the small space and very busy clinic, it is very important that children who are ill are picked up quickly to prevent further spread of illness, as well as any worsening symptoms.
Notify the clinic or front office of phone number or email changes so that you can be notified quickly.
If you have provided an email address, clinic notifications may be sent throughout the day to keep you informed if your child visits the clinic. Please check emails frequently throughout the day.
Please notify the clinic or front office if your child will be absent from school due to illness.
Please email Mrs. Renetzky, PE teacher, if you feel your child needs to sit out of PE. If a child cannot participate in PE, they will also need to sit out at recess.

All injuries resulting in PE/activity restrictions will require a doctor’s note stating dates of restrictions, as well as date of release to return to PE/activity.
